Advancing Multidisciplinary Care and Health Equity in Metastatic Breast Cancer

Explore modern metastatic breast cancer care: longer survival, smarter symptom control, caregiver support, and steps to close racial treatment gaps.

The treatment landscape for metastatic breast cancer has evolved rapidly over the past decade. As therapeutic advances extend survival, patients are living longer, transforming metastatic breast cancer into a chronic condition for many. However, this progress has also introduced new complexities in care. Optimal outcomes now depend not only on selecting effective therapies but also on delivering comprehensive, multidisciplinary care that addresses patients’ medical, psychosocial, and structural needs.

The management of metastatic breast cancer is best approached as a lifelong journey, requiring coordination across oncology subspecialties and supportive services. Treatment strategies must integrate systemic therapies—including endocrine therapy, HER2-targeted therapy, chemotherapy, antibody-drug conjugates, immunotherapy, and precision medicine testing for targeted agents—with localized palliative modalities, such as radiation or, in select cases, surgery.

Equally important is the proactive management of treatment-related toxicities and disease-related symptoms. Bone-modifying agents, analgesics, and supportive medications for neuropathy, nausea, and fatigue are essential components of care. Early referral to palliative care has become standard practice, improving symptom burden and quality of life without compromising outcomes.

Beyond pharmacologic management, lifestyle and supportive interventions play a critical role in treatment effectiveness. Referral to nutrition services can help mitigate treatment-related weight loss and maintain functional status. Structured physical activity, such as light aerobic exercise or resistance training, can improve fatigue and sleep disturbances. Psycho-oncology services provide essential support for anxiety, depression, and adjustment to chronic illness, whereas patient navigators and social workers help address logistical barriers such as transportation, financial burden, and caregiving needs.

Within this broader framework of care, the presence of an engaged support system is a key determinant of patient outcomes in metastatic breast cancer. Care partners, whether family members, friends, or other caregivers, play an essential role in helping patients navigate the complexity of treatment.

Patients with informed and involved care partners often demonstrate improved adherence to therapy, better understanding of treatment goals, and greater confidence in clinical decision-making. Care partners frequently assist with appointment coordination, medication management, and symptom monitoring, and can serve as advocates during clinical encounters. Just as importantly, they provide emotional support that mitigates isolation and psychological distress, which are prevalent in patients with advanced disease.

However, even with strong support systems and advances in treatment, significant disparities in breast cancer outcomes persist. Black women continue to experience the highest mortality rates, with up to a 41% higher risk of death compared with White women. These disparities are multifactorial, driven by a combination of structural, socioeconomic, and biological factors, including differences in access to care, lack of insurance, tumor biology, and systemic inequities.1

In metastatic breast cancer, ensuring equitable access to standard-of-care (SOC) therapies and clinical trials is critical. Despite many advances in cancer treatment, patients from racial and ethnic minority groups and medically underserved populations are less likely to receive the recommended SOC for their cancer. Historically, patients who are Black have been underrepresented in clinical research, limiting the applicability of emerging data across diverse populations.

Addressing these gaps requires a multifaceted approach. Clinicians must prioritize patient education through clear, culturally sensitive communication, which is essential to ensure patients understand not only the science behind their disease and the rationale for the treatment plan but also the goals and expectations of therapy. These discussions help identify and dispel misconceptions and address concerns based on personal or others’ experiences. Addressing misconceptions, acknowledging historical mistrust, and tailoring education to the patient’s perspective can significantly enhance engagement, trust, and adherence.

In addition, identifying and mitigating barriers to care—whether financial, logistical, or systemic—is equally important. Connecting patients with resources such as transportation assistance, financial support programs, and patient navigation services can significantly improve access and continuity of care.

Patients should also be empowered to advocate for themselves, seeking care at academic or high-volume cancer centers when possible, asking questions about treatment options, and communicating openly about barriers that may impact adherence. Many of these barriers can be mitigated through existing support programs, including financial assistance, patient navigation, and community-based resources.

As the therapeutic landscape for metastatic breast cancer continues to evolve, so, too, must our approach to care delivery. Integrating multidisciplinary support, engaging care partners, and addressing persistent disparities are not adjuncts to treatment; they are central to achieving optimal outcomes.

For patients with metastatic breast cancer, the ever-increasing technological advances in treatment and the promise of precision medicine are already being realized. The challenge now is to ensure that these advances are accessible and equitably delivered to all patients.
