Dr. Shah confirms OPTIMA was a noninferiority trial and explains that patients who avoided chemotherapy based on a favorable Prosigna score achieved outcomes statistically comparable to those on standard chemotherapy and endocrine therapy. She and Dr. Chen discuss the toxicities avoided — anthracycline-related cardiotoxicity, taxane-related neuropathy, nausea, vomiting, neutropenic fever — and Dr. Shah addresses limitations: patients under 40 weren't included, follow-up is still short at about four years, and she remains cautious in patients with four to nine positive nodes given some early crossing of survival curves.
